ABSTRACT

OBJECTIVE@#To compare the efficacy and muscle injury imaging between oblique lateral lumbar interbody fusion (OLIF) and transforaminal lumbar interbody fusion (TLIF) in the treatment of single-segment degenerative lumbar spinal stenosis.@*METHODS@#The clinical data of 60 patients with single-segment degenerative lumbar spinal stenosis who underwent surgical treatment from January 2018 to October 2019 was retrospectively analyzed. The patients were divided into OLIF groups and TLIF group according to different surgical methods. The 30 patients in the OLIF group were treated with OLIF plus posterior intermuscular screw rod internal fixation. There were 13 males and 17 females, aged from 52 to 74 years old with an average of (62.6±8.3) years old. And 30 patients in the TLIF group were treated with TLIF via the left approach. There were 14 males and 16 females, aged from 50 to 81 years old with an average of (61.7±10.4) years old. General data including operative time, intraoperative blood loss, postoperative drainage volume, and complications were recorded for both groups. Radiologic data including disc height (DH), the left psoas major muscle, multifidus muscle, longissimus muscle area, T2-weighted image hyperintensity changes and interbody fusion or nonfusion were observed. Laboratory parameters including creatine kinase (CK) values on postoperative 1st and 5th days were analyzed. Visual analogue scale(VAS) and Oswestry disability index(ODI) were used to assess clinical efficacy.@*RESULTS@#There was no significant difference in the operative time between two groups(P>0.05). The OLIF group had significantly less intraoperative blood loss and postoperative drainage volume compared to the TLIF group(P<0.01). The OLIF group also had DH better recovery compared to the TLIF group (P<0.05). There were no significant differences in left psoas major muscle area and the hyperintensity degree before and after the operation in the OLIF group (P>0.05). Postoperativly, the area of the left multifidus muscle and longissimus muscle, as well as the mean of the left multifidus muscle and longissimus muscle in the OLIF group, were lower than those in the TLIF group (P<0.05) .On the 1st day and the 5th day after operation, CK level in the OLIF group was lower than that in the TLIF group(P<0.05). On the 3rd day after operation, the VAS of low back pain and leg pain in the OLIF group were lower than those in the TLIF group (P<0.05). There were no significant differences in the ODI of postoperative 12 months, low back and leg pain VAS at 3, 6, 12 months between the two groups(P>0.05). In the OLIF group, 1 case of left lower extremity skin temperature increased after the operation, and the sympathetic chain was considered to be injured during the operation, and 2 cases of left thigh anterior numbness occurred, which was considered to be related to psoas major muscle stretch, resulting in a complication rate of 10% (3/30). In the TLIF group, one patient had limited ankle dorsiflexion, which was related to nerve root traction, two patients had cerebrospinal fluid leakage, and the dural sac was torn during the operation, and one patient had incision fat liquefaction, which was related to paraspinal muscle dissection injury, resulting in a complication rate of 13% (4/30). All patients achieved interbody fusion without cage collapse during the 6- month follow-up.@*CONCLUSION@#Both OLIF and TLIF are effective in the treatment of single-segment degenerative lumbar spinal stenosis. However, OLIF surgery has obviously advantages, including less intraoperative blood loss, less postoperative pain, and good recovery of intervertebral space height. From the changes in laboratory indexes of CK and the comparison of the left psoas major muscle, multifidus muscle, longissimus muscle area, and high signal intensity of T2 image on imaging, it can be seen that the degree of muscle damage and interference of OLIF surgery is lower than that of TLIF.

ABSTRACT

OBJECTIVE@#To observe the clinical efficacy of micro needle knife therapy on cervical vertigo.@*METHODS@#A total of 300 patients with cervical vertigo treated from January 1, 2017 to December 31, 2019 were randomly divided into micro needle knife group (96 cases, 4 cases falling off), traditional acupuncture group (96 cases, 4 cases falling off) and oral drug group(95 cases, 5 cases falling off). The micro needle knife group was treated with micro needle knife in the local upper cervical segment once every other day for 7 times; the traditional acupuncture group was treated with traditional acupuncture once a day for 2 weeks;the oral drug group was given Merislon (betahistine mesilate tablets) orally, 6 mg each time, 3 times a day for 2 weeks. The patients were followed up before treatment, at the end of treatment and 3 months after treatment. The dizziness handicap inventory(DHI) was observed and the curative effect was evaluated according to the DHI score.@*RESULTS@#At the end of the course of treatment and 3 months after the treatment, DHI scores of the three groups were significantly lower than those before treatment(P<0.01), and the DHI scores of micro needle knife group were lower than those of traditional acupuncture group and oral drug group at the same time point after treatment (P<0.01). There was no statistically significant difference in DHI scores between traditional acupuncture group and oral drug group at the same time after treatment(P< 0.05). The curative effect was evaluated according to DHI score:in micro needle knife group, 50 cases were cured, 28 cases were markedly effective, 14 cases were improved, the total effective rate was 95.83%; in traditional acupuncture group, 28 cases were cured, 26 cases were markedly effective, and 24 cases were improved, with the total effective rate of 81.25%;in oral drug group, 18 cases were cured, 20 cases were markedly effective, and 28 cases were improved, with the total effective rate of 69.47%. The total effective rate of micro needle knife group was significantly higher than that of other two groups (χ2=45.956, P=0.000). Among the cured patients in the three groups, the time required in the micro needle knife group was significantly less than that in the traditional acupuncture group and oral drug group(F=18.796, P=0.000).@*CONCLUSION@#Micro needle knife loosening atlantoaxial soft tissue has obvious curative effect on cervical vertigo which clinical cure rate, effective rate and treatment time required for cured patients are better than traditional acupuncture and oral drug.

ABSTRACT

OBJECTIVE@#To investigate the effect of ultrasound guided reduction and exploration of ulnar nerve position and percutaneous crossed pin fixation for the treatment of displaced supracondylar fracture of the humerus in children.@*METHODS@#The clinical data of 45 patients with displaced supracondylar fracture of humerus from December 2017 to December 2018 were analyzed retrospectively, including 26 boys and 19 girls, ranging in age from 1 year and 3 months to 11 years and 4 months, with an average of 7.6 years old;44 cases of crashing injury, 1 case of falling injury;29 cases on the left side, 16 cases on the right side;12 patients classified to Gartland typeⅡand 33 patients classified to Gartlandtype Ⅲ. The operation was performed from 4 h to 7 d after injury, with an average of 2.5 d. There were no neurological and vascular injuries occurred in the children. Ultrasound was used to guide the fracture reduction of the child, and the cross-needle was fixed. In the medial needle insertion, the ulnar nerve position was detected by ultrasound to avoid damage to the ulnar nerve. The ulnar nerve state was observed during operation. The clinical function evaluation criteria of Flynn was used at the latest follow-up. The evaluation criteria was used to evaluate the functional and aesthetic characteristics of the elbow joints of the children, and to observe the complications such as ulnar nerve injury after operation.@*RESULTS@#Ultrasound was used to detect the fracture from the medial side of the elbow, the lateral aspect of the elbow and the sagittal plane of the elbow. The position of the ulnar nerve could be clearly explored to avoid ulnar nerve injury when the needle was inserted inside. All children were followed up, and the duration ranged from 6 to 12 months, with an average of 9 months. None of the patients had a loss of repositioning and the fractures healed. The healing time ranged from 4 to 6 weeks, with an average of 5 weeks. At the latest follow-up, according to Flynn's evaluation criteria:compared with the healthy side, 41 patients with flexion and extension limitation were 0° to 5°, clinically evaluated as excellent;3 patients with flexion and extension limitation were 6° to 10°, clinically evaluated as good;1 patient with flexion and extension limitation was 11° to 15°, clinically evaluated as acceptable. Compared with the healthy side, 40 patients lost 0° to 5°of the angle, and 5 patients lost 6° to 10°of the angle. There were no complications such as ulnar nerve injury and cubitus varus.@*CONCLUSION@#Although ultrasound-guided treatment of displaced supracondylar fracture of the humerus in children has higher requirements for the operator at present, because of its advantages of clear development, portability, effectiveness and no impact on health, it could clearly explorethe fracture situation during the operation, guide the reduction of the fracture, and accurately show the position of the ulnar nerve, effectively improve the safety of the medial puncture, so as to minimize the complications The occurrence of the disease. Therefore, the treatment of displaced supracondylar fracture of humerus with ultrasound-guided manual reduction and percutaneous cross needle fixation is effective and worthy of further promotion.

ABSTRACT

<p><b>OBJECTIVE</b>To compare clinical results of Sanders II-IV fractures of calcaneus treated by self-setting calcium phosphate cement and allograft with locking reconstruction bone plate internal fixation.</p><p><b>METHODS</b>From March 2012 to December 2015, 48 patients with Sanders II-IV fractures of calcaneus were treated by open reduction and internal fixation through L-shape incision, bone grafting were performed on reserved bone defect of calcaneal fracture. The patients were divided into self-setting calcium phosphate cement and allograft group according to different materials. Twenty-eight patients in self-setting calcium phosphate cement group, including 23 males and 5 females aged from 22 to 52 years old with an average of (34.46±7.33) years old; 8 cases were type II, 11 cases were type III and 7 cases were type IV according to Sanders classification. Twenty patients in allograft group, including 17 males and 3 females aged from 24 to 55 years old with an average of(36.40±7.93) years old; 6 cases were type II, 7 cases were type III and 9 cases were type IV according to Sanders classification. Postoperative wound complications, Böhler angle before and after operation and 12 months, Maryland functional score were evaluated.</p><p><b>RESULTS</b>All patients obtained bone union, and were followed up from 12 to 42 months with an average of 25 months. There were no statistical significance in Böhler angle and Manyland score before and after operation and 12 months between two groups. Five patients occurred wound complications in allograft group and 2 cases occurred wound complications in self-setting calcium phosphate cement and allograft group.</p><p><b>CONCLUSIONS</b>Both of open reduction bone grafting and reconstructive plate internal fixation have good effective results for calcaneal fracture. Compared with allograft, self-setting calcium phosphate cement, which has no rejection response and less complications, has equal effects for calcaneal fracture and deserves to be popularized.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To identify the affect of chronic low back pain on multifidus muscle atrophy and fatty infiltration.</p><p><b>METHODS</b>From March 2010 to August 2013, a retrospective study were carried out in the department of orthopedics of patients with low back pain. Finally 31 cases were selected to this study including 19 males and 12 females with an average age of 36.4 years ranging from 23 to 55 years. The main symptoms of these patients were repeated back pain. Duration was more than 1 year. X-ray, CT, MRI showed no obvious abnormalities. The changes of net cross-sectional area of multifidus and T2 signal ratio of the same patient were measured at different time by MRI. VAS and Oswestry disability scores were recorded in two MRI examination. Correlation between these change of multifidus net area and T2 signal ratio in two times measurement and duration of low back pain, VAS, Oswestry disability scores were analyzed to find the affection of low back pain on paraspinal multifidus muscle.</p><p><b>RESULTS</b>The net multifidus cross-sectional area in same case by the second follow-up MRI is significantly smaller than that of the first follow-up, T2 signal ratio at second was significantly higher than that of the first (P < 0.05). The net cross sectional area of multifidus muscles reduced rate were positively correlated with VAS scores, duration and of Oswestry disabilitry scores (P < 0.001). The rate of increase in T2 signal ratio was not correlated with VAS scores,duration and the Oswestry disability scores (P > 0.05).</p><p><b>CONCLUSION</b>Chronic low back pain is one of the most important reasons of paraspinal multifidus muscle atrophy and fatty. The duration, VAS and Oswestry disability scores of chronic low back pain were positively correlated with the multifidus muscle atrophy.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the chemical components of the essential oil extracted from six cultivars' Juhua (Zaogongju, Wangongju, Huangyaoju, Chuju, Xiaoboju and Daboju) produced in Anhui province of China, which were breed from Chrysanthemum morifolium, and to provide scientific basis for quality control.</p><p><b>METHOD</b>The essential oils of the six cultivars'Juhua were extracted by water-steam distillation and separated by GC capillary column chromatography. The components were quantitatively determined with normalization method, and identified by GC-MS.</p><p><b>RESULT</b>From Zaogongju, Wangongju, Huangyaoju, Chuju, Xiaoboju and Daboju, 247, 226, 246, 182, 216 and 122 chromatographic peaks were detected, among them 75, 54, 78, 50, 53 and 43 components were identified,which were composed of 85.67%, 82.80%, 81.38%, 73.22%, 71.51% and 72.87% of the total essential oil, respectively.</p><p><b>CONCLUSION</b>Monoterpenoid compounds were higher more than sesquiterpenoid compounds in the five juhua except for Chuju. There was no difference in the constituents of essential oil of Zaogongju and Wangongju. And verbenyl acetate was the main chemical constituent and composed of 32.10% and 37.85% of the total essential oil, respectively. (1R)-Camphor and bisabolol oxide A were the main chemical constituents in Huangyaoju, which were composed of 28.70% and 12.58% of the total essential oil, respectively. beta-Selinene and borneol were the main chemical constituents in Chuju, which were composed of 17.85% and 12.84% of the total essential oil, respectively. Eucalyptol was the main chemical constituent in Xiaoboju, which were composed of 21.33%. Verbene oxides and chrysanthenone were composed of 25.32% and 8.26% of the total essential oil, respectively, in the Daboju. The common chemical constituents were camphene, borneol, bornyl aceate, (1R)-camphor, (-)-4-terpineol, alpha-terpineol, eucalyptol, cis-caryophyllene, caryophyllene oxide, juniper camphor, beta-sesquiphellandrene, alpha-curcumene and beta-farnesene in the six cultivars' Juhua produced in Anhui province of China. The resultant data provided the further evidences to distinguish Juhua' cultivars and scientific basis for quality control of Juhua.</p>

ABSTRACT

<p><b>OBJECTIVE</b>Botanical character of different cultivated types medicinal Chrysanthemum morifolium were observed and compared, which can offer reference for its genetic diversity research and breeding.</p><p><b>METHOD</b>Based on the random blocks field experiments design, the main character of leaf and capitulum of medicinal C. morifolium were observed, it was studied on that the botanical character of different cultivated types medicinal C. mortfolium in China were comparatively study with statistical means.</p><p><b>RESULT AND CONCLUSION</b>There are some different of botanical character among different cultivated types medicinal C. morifolium in China, which can be divided into 7 colonies according as its diversity of the botanical and morphological character. The sawtooth number of lamina, leafstalk length and leaf division number are the important laminae morphologic character of different types medicinal C. morifolium, which are relative independent characters.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the chemical components of the essential oil extracted from five cultivars' Hangjuhua (Zaoxiaoyangju, Chixiaoyangju, Dayangju, Yizhongdabaiju and Xiaotangju), which were breed came from the Chrysanthemum morifolium Ramat, and provide scientific basis for quality control.</p><p><b>METHOD</b>The essential oil of five cultivars' Hangjuhua were extracted by water-steam distillation and separated by GC capillary column chromatography. The components were quantitatively determined with normalization method, and identified by GC-MS.</p><p><b>RESULT</b>From the Zaoxiaoyangju, Chixiaoyangju, Dayangju, Yizhongdabaiju and Xiaotangju, 179, 167, 209, 211 and 270 chromatographic peaks were detected, among them 54, 56, 60, 55 and 65 components were identified, which were composed of 73.40%, 64.14%, 65.19%, 53.55% and 62.89% of the total essential oil, respectively.</p><p><b>CONCLUSION</b>The common chemical constituents were juniper camphor, 1, 3, 3-trimethylcyclohex-1-ene-4-carboxaldehyde, borneol, alpha-curcumene, alpha-bisabolol, cis-caryophyllene, benzyl benzoate, methyl beta, beta-dimethylbenzenepropanoic acid ester, 2, 4-decadienal and heneicosane in the essential oil of the five cultivars' Hangjuhua. Among them, juniper camphor is most which were composed of 11.96%, 10.51%, 10.95%, 13.28% and 10.77% of the total essential oil, and methyl beta, beta-dimethylbenzenepropanoic acid ester is second, which were composed of 1.51%, 1.86%, 2.52%, 4.89% and 2.51% of the total essential oil, respectively. There are marked differences in the compounds contained in the five samples. The juniper camphor is a characteristic constituent in the essential oil of Hangjuhua. The resultant data provided the evidences to establish Hangjuhua' cultivars and scientific basis for quality control of Hangjuhua.</p>

ABSTRACT

<p><b>OBJECTIVE</b>Analysis of the constituents of the essential oil extracted from the Xiaoboju, one of commercial breed came from the flowers of the Chrysanthemum morifolium, processed by the aeration-desiccation and desiccation after the sulfur-bumin fumigation, and to provide scientific basis for quality control.</p><p><b>METHOD</b>The essential oil was extracted by water-steam distillation and separated by GC capillary column chromatography. The components were quantitatively determined with normalization method, and identified by GC-MS.</p><p><b>RESULT</b>From the aeration dried sample and the dried sample after sulfur-burnin fumigation, 216 and 211 components were detected, among them fifty and six-five components were identified, which were composed of 73.21% and 82.32% of the total essential oil, respectively.</p><p><b>CONCLUSION</b>The yield of the essential oil extracted from the aeration dried sample was 3.50%, and that from the dried sample after sulfur-bumin fumigation was 4.22%. The latter is 1.2 times higher than the former. The components of the essential oil of both samples are mostly monoterpenoids and secondly sesquiterpenoids compounds, but there are marked differences between the compounds contained in the two samples. Therefore, the processing of flowers of the C. morifolium should be strictly controlled and standardized.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the genetic diversity of Chrysanthemum morifolium at the level of molecular biology.</p><p><b>METHOD</b>The total genomic DNA was extracted from medicinal chrysanthemums by 2% CTAB method. And the genetic diversity of 22 C. morifolium accessions was tested by RAPD marks. The NTSYS software was used to analyze the marks.</p><p><b>RESULT</b>26 10-mer arbitrary primers were found to acquire polymophic results. A total of 233 bands were amplified, of which 89.7% bands were found to be polymophic. 8.04 polymophic bands were amplified by each primer on the average. The results of cluster analysis by using UPGMA method showed that all the tested accessions could be differentiated by RAPD marks.</p><p><b>CONCLUSION</b>There actually existed much genetic diversity at the molecular level among the germplasm resources of C. morifolium. RAPD marks could be effective tools to construct DNA fingerprintings of C. morifolium. The differences between the tested chrysanthemums are related to the environments. However, it was affected by genetic facters more significantly.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical applications of portable spirometry in asthma.</p><p><b>METHODS</b>Twenty patients with asthma were recruited from Peking Union Medical College Hospital. Flow-volume loop, simultaneous asthma symptoms, and mood were monitored three times a day for consecutive 14 days.</p><p><b>RESULTS</b>In patients with a normal daytime spirometry, marked decline of forced expiratory volume in one second (FEV1) and peak expiratory flow (PEF) were observed at night and/or in the early morning. A within subject correlation analysis between FEV1, PEF, and asthma symptoms showed that the correlation between symptoms and airway obstruction was found only in seven out of twenty patients (35%). Four patients (20%) reported many symptoms with nearly normal portable spirometry. Accordingly, their symptoms were not correlated with FEV1 and PEF. This group of patients was defined as over-perceivers. On the contrary, another two patients (10%) did not report any symptoms while obvious airways obstruction was recorded by a portable spirometry. These patients were defined as under-perceivers.</p><p><b>CONCLUSIONS</b>Dynamic monitoring of flow-volume loop with a portable spirometry is more accurate than routine lung function test in assessment of asthma severity. In addition, combined with simultaneous monitoring of symptoms, it would be of particularly helpful in identifying two specific types of asthma patients, e.g. over-perceivers and under-perceivers.</p>

<p><b>OBJECTIVE</b>To study the quality and genetic diversity of Chrysanthemun morifolium, the chemical constituents of 20 medicinal chrysanthemum cultivars in China were compared.</p><p><b>METHOD</b>Chemical constituents of the 20 cultivars were compared in three types of index: chlorogenic acid, flavonoid and volatile oil.</p><p><b>RESULT</b>There are visible differences on the contents of chlorogenic acid, flavonoid and volatile oil between the cultivars.</p><p><b>CONCLUSION</b>Each cultivar has its own internal quality, and it is affected by many factors. The chemical constituents of the cultivars showed the genetic diversity in medicinal chrysanthemums of China.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate botanical characters and yield of four cultivars of Chrysanthemum morifolium for further study on their genetic diversity and selective breeding.</p><p><b>METHOD</b>The characters were observed and yield was investigated by field randomized block and analysis of variance.</p><p><b>RESULT AND CONCLUSION</b>The botanical characters are difference among four cultivars; the amount of single flower head is the main factor influencing on the output of Chrysanthemum morifolium (r = 0.925); the yield of "Hongxinju" and "Xiaobaiju" are remarkably higher than that of "Dabaiju" and "Changbanju".</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate salt tolerance of seedlings of 4 medicinal C. morifolium cultivars to be transplanted, and to expand the planting area.</p><p><b>METHOD</b>Seedlings were cultivated in hoagland nutrient solution containing various concentrations of NaCl for 30 days. The height, dry weight and chlorophyll content were investigated. Identification index mainly in relative growth rate, the evaluation of NaCl effects on the growth, K+, Na+ and Cl- distribution in seedlings were surveyed.</p><p><b>RESULT AND CONCLUSION</b>The salt tolerance was difference among four cultivars of C. morifolium. The salt tolerance of "Dabaiju" and "Changbanju" was weak, while "Hongxinju" and "Xiaobaiju" was strong. "Hongxinju" and "Xiaobaiju" may be planted in salinte soil area.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study organochlorine pesticide and heavy metal residuals of Chrysanthemum morifolium.</p><p><b>METHOD</b>The contents of organochlorine pesticide residuals was determined by chromatography, Cu, Pb, Cd was determined by AAS, and As, Hg was determined by AFS.</p><p><b>RESULT AND CONCLUSION</b>The contents of organochlorine pesticide and heavy metals residuals are up to "Green Trade Standards of Importing & Exporting Medicinal plants & Preprations" (MOFTEC, 2001), but the contents are different among four cultivars clearly.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the diagnosis and therapy of systemic lupus erythematosus (SLE) patients with pulmonary thromboembolism (PTE).</p><p><b>METHODS</b>11 hospitalized cases were reviewed retrospectively in PUMC Hospital during January 1984-July 2001.</p><p><b>RESULTS</b>All 11 cases were suffered from severe active lupus with PTE. The SLE-DAI (SLE-disease active index) was 21.9 +/- 4.9. 7 cases had first onset of progressive Raynaud phenomenon. Anti-RNP antibody was positive in 73% of the cases. Echocardiogram revealed medium-severe pulmonary hypertension. When PTE was found, while 6 cases had started with smaller to medium dose of prednisone treatment, which was 20-30 mg/d, and other 4 cases received no prednisone. Only 1 received large dose of prednisone and immunosuppressor. Large dosage of prednisone, immunosuppressor with or without anticoagulant were given to those 6 and 4 patients after final diagnosis, respectively. 6 of 7 cases showed relieved Raynaud phenomenon while 4 cases hemoptysis were disappeared, echocardiogram had confirmed that pulmonary artery pressure decreased in 4 cases. [(31.7 +/- 12.4) mmHg]. 5 cases survived, 3 were dead and 3 failed to be followed up.</p><p><b>CONCLUSIONS</b>Patients of SLE with PTE are liable to be misdiagnosed or missed-diagnosed, The risk factors are active-SLE, progressive Raynaud phenomenon, and symptoms of thromboembolism, positive anti-RNP antibody and mild-medium pulmonary artery hypertension. Combined therapy of present available measures like large dose of prednisone, immunosuppressors and anticoagulant are highly recommended.</p>
